At Home Health of Western Colorado, we’re a Medicare/Medicaid-certified home health care agency – a friendly and experienced team of nurses, rehab therapists and home health aides dedicated to providing your individualized health care needs with compassion and empathy.

Our professionals are specially trained in caring for older adults, and provide in-home services that can be a lower-cost alternative to inpatient care, and can decrease the likelihood of a return to the hospital. In collaboration with you and your physician, your home health nurse develops a plan of care designed to nurture your health and, and help maintain your independence.
