  • History and physical
  • Face Sheet
  • Recent labs
  • 48 hours of nurses notes
  • Any psychiatric history or assessments
  • Medication list
  • Copy of Medicare and/or insurance Card
  • Copy of picture identification
  • Guardianship/Healthcare POA papers if applicable
