Assisted Living Features

Your individualized plan of care begins with a health assessment by a registered nurse from Sleepy Eye Area Home Health. Together you’ll select personal care support that suits needs and preferences. A registered nurse coordinates the care services you choose.

  • Physical and occupational therapy, and speech/language pathology (outpatient)

Sleepy Eye Area Home Health (assisted living services), accepts private pay; long-term care insurance; Community Access for Disability Waiver (CADI) and MN Elderly Waiver (EW).
